Privacy Act Statement
Collection of the information is authorized by the Freedom of Information Act (FOIA), 5 U.S.C. § 552, and the Privacy Act of 1974, 5 U.S.C. § 552a, as amended. The primary purpose of this information collection is to identify responsive records, verify requesters’ identities when required by law, determine the appropriate processing track, and provide the requested records. DOI may also use the information in carrying out other responsibilities arising from the administration of FOIA and Privacy Act requirements.
Information you provide may be disclosed to another Federal agency when necessary to consult on disclosure determinations, to refer records for review, or to obtain subject matter expertise. For FOIA requests, DOI may include limited request information in publicly available FOIA logs. Privacy Act request information is not included in public logs and is protected in accordance with applicable Systems of Records Notices (SORNs).

Furnishing the requested information is voluntary; however, failure to provide necessary details may result in delays or prevent DOI from processing your FOIA or Privacy Act request.
